You’ve probably seen the videos making this claim, that one exercise will eliminate your patellar tendon pain for good. The hype traces back to a study by Rio et al. which found that heavy isometric exercise reduced patellar tendon pain for up to 45 minutes post-exercise. Cue the Spanish squat becoming everyone’s go-to recommendation. Sounds like we solved tendon pain, right? Not quite. A follow-up study failed to replicate those results. In a larger sample, the pain response was all over the place. Some people improved, some stayed the same, and some actually hurt more. Isometrics can absolutely have a place in a structured tendon rehab program.
